Forms to Download
During your debt settlement process, you may need to fill in certain forms.
Find out and download the most frequently used forms.
For third-party authorisation
If you want to authorise someone with regard to issues relating to your debt settlement, you may download, fill in and sign:
- General authorisation form (General authorisation form for debt included in the Eurobank and the Frontier portfolio) – you will need to state the reason why you authorise the third party and have your signature authenticated by a police department or a Citizen Service Centre (KEP).
Alternatively, you may issue an authorisation through gov.gr, following the step-by-step instructions. It will be legally equivalent to an authorisation with authenticated signature.
For settlements pursuant to the Code of Conduct
If you want to settle your debt pursuant to the Code of Conduct under Law 4224/2013, you need to log in the Code of Conduct digital platform within the specified deadline and submit, exclusively through the platform, a debt settlement application, filling in the platform fields accurately and fully with the information regarding your financial situation as well as your assets and liabilities, including those found in the following forms:
- STANDARDISED FINANCIAL STATEMENT (only available in Greek) – SUPPORTING DOCUMENTS OF INDIVIDUALS FOR ASSESSMENT OF FINANCIAL SITUATION (LAW 4224/2013)
- FINANCIAL INFORMATION FORM (only available in Greek) - SUPPORTING DOCUMENTS OF COMPANIES FOR ASSESSMENT OF FINANCIAL SITUATION (LAW 4224/2013)
You will be able to post the above documents by submitting your application for the settlement of your debts on the Digital Platform of the Code of Conduct. For lodging an appeal
If you want to lodge an appeal pursuant to Law 4224/2013, download the form you need to fill in and sign:
- Borrower appeal form (only available in Greek) to lodge your appeal to doValue Greece.
For processing of special categories of personal data
If you are facing health problems, you need to submit the relevant documents. These include personal health data (e.g. for physical and mental condition, disabilities, medical history or other health data).
For these documents to be accepted, you must be informed and give your consent to the processing of your special category personal data. Download the form you need to fill in and sign:
If the health problem concerns a third party connected to you, they also need to download, fill in and sign:
- Solemn declaration – consent authorising you to submit the relevant documents on their behalf.
You need to have your signature authenticated by a police department or a Citizen Service Centre (KEP).
Alternatively, you may issue a solemn declaration through gov.gr, following the step-by-step instructions. It will be legally equivalent to a solemn declaration with authenticated signature.

It is noted that in case you have authorized a third individual, please attach with the power of attorney document (notarial power of attorney or authorization) the corresponding Solemn Declaration "SD for non-revocation of power of attorney" signed (with a certificate of authentic signature either by KEP or other public authority or via e-gov).
- Declaration 1 of the Regulation on non-revocation of a power of attorney (not more than 3 years from the drafting of an initial power of attorney)
- Declaration 2 of the Regulation on non-revocation of a power of attorney (3 years have elapsed since the initial power of attorney)
- Declaration 3 of the Regulation on non-revocation of power of attorney (renewal of opinion)
